Record Keeping:
In keeping with standards of professional practice, I will keep brief records of your sessions, phone calls, emails or inquiries. These records are kept to ensure a direction to your sessions and continuity in service. Your personal information will be collected, used and stored as necessary to ensure the provision of high-quality services and in accordance with the requirements of relevant privacy laws. All case notes are kept secure and confidential in a locked file cabinet, on a secure computer which requires a password for access, or on Jane which requires a password for access. They will not be shared except with respect to the limits to confidentiality. Should you wish to have your records released, you are required to sign a release of information, which specifies what information is to be released, and to whom. Jane has been specifically developed for delivery of healthcare. Jane is PHIPA and PIPEDA compliant with secure end to end 256 bit encryption, with data stored on Canadian servers. Although your record remains the property of Kelsey Feinstein Counselling, you understand that you have the right to access the personal information complied pertaining to your information by submitting a written request. If you would like access or request a correction to a record please provide a written request to me via email. I must respond to your request for access to your record within 30 days. Client files are stored for 10 years (10 years following the 18th birthday if the client is under the age of 18). If you have to make a complaint concerning how I handle your personal health information in your records please contact the Office of the Information and Privacy Commissioner of Ontario.
